Treating crepey skin and loss of elasticity after GLP-1 weight loss
Volume loss and sagging get most of the attention after weight loss. Skin quality gets less, and it is often the change clients find hardest to describe. The skin looks thin. It creases in fine parallel lines when you move. It catches the light differently than it used to.
This is a different problem from loose skin, and it needs a different set of treatments. Tightening will not fix texture, and filling will not fix thinness. What crepey skin needs is rebuilding from within the dermis.
What is crepey skin
Crepey skin is skin that has lost thickness and elastic recoil. It looks finely wrinkled, similar to crepe paper, and the lines appear across the surface rather than in the deeper folds you would associate with expression.
It shows up most on the neck, chest, upper arms and inner thighs, and across the cheeks where the skin is thinnest. Clients often notice it first in indirect light or in photographs rather than in the mirror.
The distinction that matters clinically: loose skin is a volume and support issue, crepey skin is a dermal quality issue. They frequently occur together after weight loss, and they respond to different treatments. Getting the diagnosis right is most of the work.
Why skin does not snap back
This is the question we hear most often, and the honest answer involves two proteins that behave very differently.
Collagen provides thickness and strength. Your body rebuilds it throughout life, and we can prompt it to rebuild more. Elastin provides recoil, the property that lets skin return to its original position after being stretched. Adults produce very little new elastin, and the elastin fibres you have degrade with age, sun exposure and mechanical stretch.
So when skin has been stretched over a larger body for years and that volume disappears in a matter of months, the collagen can be replaced but the elastic framework largely cannot. The skin has no mechanism to fully contract back.
Rapid weight loss makes this worse in a second way. Reduced food intake on a GLP-1 medication means less protein and fewer micronutrients available for tissue repair, at exactly the point when the skin is under the most strain.
What treatment can and cannot do
We are straightforward with clients about this. We cannot restore elastin. What we can do is rebuild collagen, which thickens the dermis, improves how the skin reflects light, and softens the crepey appearance considerably.
In practice that means the texture improves, the skin looks and feels denser, and fine surface lines soften. What it does not mean is that skin which has been substantially stretched will return to its pre-weight-loss state. Someone who has lost a great deal of weight over a short period will see meaningful improvement rather than reversal.

Timing matters more here than anywhere
In our clinical experience, patients respond considerably better to collagen-stimulating treatments when they are treated within roughly the first three months of starting a GLP-1 medication.
After that, the collagen response appears to diminish, likely due to the hormonal shifts these medications produce and lower nutritional intake as appetite drops.
For skin quality specifically, this window is worth taking seriously. Collagen built while the skin is still stretched holds the dermis together as volume comes off. Building it afterwards means working against a thinner starting point.
If you are already past three months, treatment still helps. It simply takes more sessions to reach the same place.

Why has my skin not tightened up after losing weight?
Elastin is the protein that gives skin its recoil, and adults produce very little new elastin. Once those fibres have been stretched and degraded, the skin has no way to fully contract on its own. Collagen can be rebuilt, which is what treatment targets.
Is crepey skin the same as loose skin?
