Treating jowls and jawline laxity after GLP-1 weight loss
A softening jawline is one of the first changes clients raise with us after losing weight on a GLP-1 medication. The face is slimmer everywhere else, and yet the lower third looks heavier and less defined than it did before.
It is a frustrating result, and it makes sense once you understand what has actually happened. Jowling after weight loss is rarely a skin problem on its own. It is a support problem, and treating it as though it were only loose skin is why a lot of people are disappointed with their results.
Why jowls appear after losing weight
The jawline holds its shape because of what sits above it. Volume through the cheeks and midface lifts the surrounding tissue and keeps it suspended. Take that volume away quickly and the tissue has nothing holding it up, so it settles downward along the jaw.
Skin elasticity is the second factor. Skin contracts, but slowly, and it does not keep pace with rapid weight loss. The faster the loss, the wider the gap between how much tissue has gone and how much the skin has been able to draw back.
Age matters here. Collagen and elastin decline steadily from our thirties onward, so the same amount of weight loss produces more jowling at 48 than it does at 32.

The mistake most people make
The instinct is to treat jowls as loose skin and go straight to a tightening device. Tightening alone can improve the picture, but it works against gravity without addressing why the tissue descended in the first place.
We approach it the other way around. Rebuild support through the mid and upper face first, then tighten, then refine the jawline itself with a small amount of filler if it is still needed. Working in that order means each treatment does less work and the result holds better.
It also means most clients need less product overall, which matters both for how natural the outcome looks and for what it costs.
Timing changes what is possible
If you are early in a GLP-1 course, you have more options than someone who finished eighteen months ago.
In our clinical experience, patients respond considerably better to collagen-stimulating injectables when treated within roughly the first three months of starting a GLP-1 medication. After that, the collagen response to Sculptra and Radiesse appears to diminish, likely because of the hormonal shifts these medications produce and reduced nutritional intake as appetite drops.
Building collagen while the skin is still capable of a strong response means less descent to correct later. Prevention is a far easier project than correction, and the jawline is where that difference shows up most visibly.
If you are already past that point, treatment still works. We simply lean harder on energy-based tightening and set expectations accordingly.
Treatments we use for jowls and jawline laxity
Sofwave
Our primary treatment for jawline laxity. Sofwave delivers ultrasound energy at 1.5mm into the mid-dermis, generating heat that triggers new collagen formation. It is FDA-approved and Health Canada-approved for lifting the eyebrow, submental area and neck, and for improving facial lines and wrinkles.
One session, roughly 30 to 45 minutes, no downtime. Results develop over about three months as collagen rebuilds. Most clients need a single treatment per year.
Sculptra
Poly-L-lactic acid, injected to stimulate your own collagen through the cheeks and midface. This is the support piece. Restoring structure above the jawline lifts the tissue that has descended, which does more for jowling than treating the jawline in isolation.
Results build over three to six months and hold for up to two years or more.
Sylfirm X
Radiofrequency microneedling for skin quality and firmness. Where the skin along the jaw and lower face has become thin or crepey alongside the laxity, Sylfirm X addresses the texture side of the problem. We were the first clinic in Ontario to offer it.
Dermal Fillers
Hyaluronic acid filler placed along the jawline to restore definition, used last and conservatively. Once the tissue has been lifted and tightened, a small amount of product in the right place sharpens the line. Placed first, it tends to add weight to an area that is already heavy.
How we usually sequence it
Every plan starts with a complimentary consultation, where we assess how much of the change is volume loss, how much is skin laxity, and how your skin is behaving..
A common sequence:

- Sofwave for lifting and tightening along the jaw and under the chin, once collagen stimulation is underway.

Why did I get jowls after losing weight when I did not have them before?
Volume through your cheeks and midface was holding that tissue up. Losing it quickly removed the support, and the skin has not contracted fast enough to compensate. Age and baseline skin elasticity determine how pronounced the effect is.
Will jowls after weight loss go away on their own?
Some skin retraction happens naturally over the first year, but it is usually partial. Tissue that has descended does not return to its original position without treatment.
Can Sofwave fix jowls without surgery?
Sofwave lifts and tightens mild to moderate laxity and works well for most people in this situation. It is not equivalent to a surgical result, and we will tell you honestly at your consultation if we think your degree of laxity is beyond what a non-surgical approach can address.
